Please note: This form is to be used to Register and Pay in Installments.  Please note that ALL payments will be treated as a deposit that is used for the Registration of the Conference/Seminar you select.  Partial registration payments cannot be applied to future conferences or events!  If you have paid enough installments to equate to FULL registration for your selected conference and must cancel at least 30 days prior to your event, we will refund your money.  However, if you cancel less than 30 days before your event or have not paid enough installments to complete a FULL registration, you will forfeit your payment and it WILL NOT BE REFUNDED TO YOU!
